$80K - $101K a year Home Health therapists provide professional care to patient/family/caregivers to ensure that the clients will function at their personal best, as prescribed by the physician and in compliance with all applicable regulations and Company policies.

Role and Responsibilities Identify and meet patients' goals and needs Offer cost-effective treatments that help improve clients' mobility and independence Develop care plans using a variety of treatment techniques Provide quality, personalized and evidence-based care and proven interventions Motivate patients during treatment in order to help them function optimally Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination Consult and practice with other health professionals Evaluate effects and monitor and communicate progress Document patient care services Qualifications and Education Requirements Must Possess a current, unencumbered, active professional license for the State of UT Electronic charting proficiency Current CPR certification Valid driver's license Preferred Skills 1 year of Home Health Experience Knowledge of OASIS charting Proven work experience as an Occupational therapist in home health Ability to manage patients with different types of personalities Current knowledge of treatment practices and Medicare LCD guidelines Professional Excellent interpersonal communication skills Valid license to practice as an Occupational therapist Graduate degree or greater in Occupational therapy.
