Job description First Choice Home Health & Hospice is seeking to hire a Full-Time Home Health Care Coordinator to help manage the care for Home Health patients.
Work includes coordinating clinical schedules, communicating with physician offices, communicating with patients, and obtaining prior authorizations from insurance companies.
Pay and Benefits $17.

Responsibilities:
Ensure that patients are onboarded correctly communicate needs to clinicians coordinate schedules for patient visits ensure proper paperwork is completed Coordinate care for patient Schedule patient visits Communicate with physician offices Obtain authorizations from insurance companies Support the clinical care team Assist clinicians by providing information and other support Help clinicians stay organized Attend team meetings Answer questions.
Estimated Salary: $20 to $28 per hour based on qualifications.
