Certified Diabetes Educator
A successful candidate for this job will provide leadership and training for Level 1 Educators. Direct and develop internal diabetes patient education and assessment programs. Work directly with internal CareCenter staff to implement said programs through the medium of tele-health. Guide patients in setting individualized behavioral and clinical goals. Develop successful metrics. Use behavior change methodology to facilitate and influence patient participation in the education process. Assist in the assessment of the program and its goals. Participate in all quality assurance activities. Collect and compile data for the Diabetes Education Program. Participate in staff development activities. Work directly with third party administrators to design population specific Diabetic Education Self Management and Training (DSME/T) programs.
